Appointment to Extension Ministry Forms

Local Church RelationshipAll clergy appointed in extension ministries shall establish membership in a charge conference in their home annual conference in consultation with the pastor in charge and with approval of the District Superintendent and the Bishop. (¶ 3443.1 of the 2016 Book of Discipline)

Appointment to Extension Ministries Form: Those appointed to an extension ministry shall submit annually to the Bishop, the District Superintendent, and the Board of Ordained Ministry a written report on the official Appointment to an Extension Ministry form developed by the GCFA. This completed form serves as the basis for their evaluation in light of the missional needs of the Church and the fulfillment of their licensing or ordination responsibilities. (¶ 344.2.a of the 2016 Book of Discipline). You are to send copies to 1. The Bishop’s Office (Jill Philipp, jphilipp [at] wocumc.org)2. Your District Superintendent; 3. The Board of Ordained Ministry (Donnetta Peaks, dpeaks [at] wocumc.org); 4. The Conference Secretary (Chris Clough, cclough [at] wocumc.org); 5. The Bishop of the area in which you serve, if other than the area of which you are a member. A copy of this report may be used to inform the Charge Conference(s) of which you are a member and an affiliate member in keeping with ¶316.1 and 344.3 a,b. A copy of this report should be used to inform the umea [at] gbhem.org (United Methodist Endorsing Agency), PO Box 340007, Nashville, TN 37203- 0007 in keeping with ¶344.1b.
